This Membership Service Agreement (“Agreement”) governs participation in the Union Puppy Club program, a collaborative membership offered by Born to Be Dog Training LLC and Heads and Tails Pet Salon LLC (collectively referred to as “Providers”).
By enrolling in Union Puppy Club and submitting this form, the undersigned dog owner (“Client”) agrees to the following terms and conditions:
Union Puppy Club is a recurring monthly membership that provides access to training and grooming benefits offered by the Providers. Membership benefits may include:
Training Benefits (Born to Be Dog Training LLC):
Weekly puppy social (reservation required)
Member-exclusive events
Access to online training courses and educational materials
Grooming Benefits (Head & Tails Pet Salon LLC):
Complimentary monthly bath
Complimentary grooming add-ons per grooming visit (blueberry facial, paw balm, teeth brushing)
10% discount on merchandise in-store
Membership benefits apply only to the enrolled dog and are non-transferable. Unused benefits do not roll over unless otherwise stated.
Enrollment in Union Puppy Club provides access to membership benefits; however, participation in certain services is subject to additional conditions.
A. Group & Social Activities
Participation in puppy socials, member events, or other in-person group activities is voluntary. Providers reserve the right to deny or remove a dog from any group activity if health or behavior presents a safety concern. To participate in group activities, Client agrees that their dog:
Is current on age-appropriate vaccinations
Is free from contagious illness
Is not a female in heat at the time of attendance
Is behaviorally appropriate for structured group interaction
Providers reserve the right to request updated veterinary documentation or proof of vaccination at any time as a condition of continued participation in group activities.
B. Grooming Services
Grooming benefits are provided by Heads and Tails Pet Salon LLC and remain subject to the salon’s standard policies, procedures, and health requirements. Membership does not override salon policies.
C. Born to Be Online Training & Educational Materials
Access to digital courses and educational materials is provided for informational purposes only. No specific behavioral outcomes are guaranteed.
A. Puppy Socials
Puppy socials require advance reservation.
Class sizes are capped at eight (8) dogs to maintain safe, structured group sizes.
Dogs must be healthy and behaviorally appropriate for group play.
All dogs must be accompanied by a handler at all times during Puppy Socials; this is not a drop-off or daycare service.
Handlers are expected to follow staff instructions at all times to support a safe and positive environment.
Providers reserve the right to remove and/or separate a dog from group activities if necessary for safety.
B. Member Events
Certain member-exclusive events may require advance RSVP. Event details, participation requirements, and capacity limits will be communicated as events are announced.
A. Payments
Union Puppy Club operates as a recurring monthly subscription. By enrolling, Client authorizes automatic monthly billing to the payment method provided. Membership benefits begin upon purchase and/or renewal and remain active through the current billing period. If a payment method fails, membership benefits may be suspended until payment is successfully processed.
B. Refund Policy
All subscription payments are non-refundable. Refunds will not be issued for any reason, including but not limited to:
Unused benefits
Partial billing periods
Missed socials or events
Mid-cycle cancellations
Failure to cancel prior to a renewal date
Inability to attend services during an active billing period
Charges processed after a renewal date
C. Cancellations
Client may cancel membership at any time. Cancellation will take effect at the end of the current billing period. No cancellation fees apply. Client is responsible for canceling prior to the renewal date to avoid additional charges. Upon cancellation, access to all membership benefits — including puppy socials, member events, grooming perks, discounts, and digital resources — will terminate at the end of the active billing period.
Members may cancel their membership at any time by logging into their membership account and selecting the cancellation option, or by emailing Born to Be Dog Training LLC at [email protected] with a cancellation request. Cancellation will take effect at the end of the current billing period.
Union Puppy Club communications, event announcements, and updates will primarily be shared within the designated membership platform. Providers may also communicate via email and/or text message at their discretion; however, delivery through those channels is not guaranteed.
Client is responsible for providing accurate contact information and for monitoring communications related to membership benefits, RSVPs, scheduling, events, and updates. Failure to review posted updates does not exempt Client from membership policies, event requirements, or billing terms.

Providers respond to non-emergency inquiries during normal business hours. Union Puppy Club does not provide emergency behavioral or veterinary services.
A. Assumption of Risk
Client understands that participation in in-person services, including puppy socials, events, and grooming services, involves inherent risks, including but not limited to: dog-to-dog interactions, minor injuries (scratches, mouthing, play-related contact), serious bodily injury, illness exposure, unpredictable animal behavior, stress, or overstimulation. Client voluntarily and knowingly assumes all risks associated with participation in Union Puppy Club services, whether such risks are known or unknown, foreseen or unforeseen.
B. Release of Liability
Client agrees to release, waive, and hold harmless Born to Be Dog Training LLC and Head and Tails Pet Salon LLC, including their respective owners, members, officers, employees, independent contractors, agents, volunteers, representatives, successors, and assigns (collectively, the “Released Parties”), from any and all claims, demands, actions, causes of action, injuries, damages, losses, liabilities, costs, or expenses (including attorney’s fees), including claims arising from the ordinary negligence of the Released Parties, to the fullest extent permitted by law, whether known or unknown, foreseen or unforeseen, arising out of or related to:
Participation in Union Puppy Club activities
Use of membership benefits
Dog-to-dog or dog-to-person interactions
Grooming services
Events occurring on or off premises used for program activities
This release does not apply to claims arising from gross negligence or willful misconduct of a Released Party. Client remains financially responsible for any injury or damage caused by their dog. Client retains full ownership, care, custody, and control of their dog at all times during services and agrees to take full responsibility for their dog’s behavior and supervision. Client acknowledges that no dog should ever be left alone with a child and that full, awake adult supervision is required at all times when children and dogs are together. Client assumes full responsibility for the supervision and safety of any minor accompanying them to Union Puppy Club activities and releases the Released Parties from any claims arising from a minor’s presence or participation.
Each Provider remains independently responsible for the services they directly deliver. Nothing in this Agreement shall be construed to create a partnership, joint venture, agency relationship, or employer-employee relationship between the Providers beyond the collaborative offering of Union Puppy Club membership benefits.
C. Indemnification
Client agrees to indemnify, defend, and hold harmless the Released Parties from and against any and all third-party claims, including claims brought by other members or participants, demands, actions, damages, liabilities, losses, judgments, settlements, and expenses (including reasonable attorney’s fees) arising out of or related to:
Injury or damage caused by Client’s dog
Client’s participation in Union Puppy Club activities
Client’s violation of this Agreement or posted program policies
Client’s negligent, reckless, or wrongful acts or omissions
This indemnification obligation applies regardless of whether a claim is brought directly against a Released Party or jointly against Client and a Released Party, except to the extent caused by the gross negligence or willful misconduct of a Released Party.
Failure to comply with the terms of this Agreement, including but not limited to health requirements, behavior expectations, payment obligations, or salon policies, may result in suspension or termination of membership at the discretion of the Providers, without prior notice if necessary for safety.
Providers reserve the right to suspend or terminate participation in specific services, including group activities, if a dog’s health, behavior, or handler conduct presents a safety concern.
Termination due to breach of this Agreement does not entitle Client to a refund of any fees paid. Providers may also suspend membership benefits in the event of failed or disputed payments until the issue is resolved.
A. Governing Law
This Agreement shall be governed by the laws of the State of New Jersey, without regard to its conflict of law principles.
B. Venue
Any disputes arising under this Agreement shall be resolved in the state or federal courts located in Union County, New Jersey.
The Providers shall not be liable for any delay, interruption, modification, or failure to perform services or provide membership benefits due to causes beyond their reasonable control, including but not limited to acts of God, severe weather, natural disasters, fire, flood, public health emergencies, government orders or restrictions, utility failures, labor disputes, facility closures, or other unforeseen circumstances. In the event of such circumstances, Providers may modify, reschedule, suspend, or temporarily limit services as reasonably necessary. Such circumstances shall not entitle Client to a refund of membership fees. Providers will make reasonable efforts to resume services as soon as practicable.
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Providers reserve the right to update or modify this Agreement or membership policies with reasonable notice to members. Continued participation after such notice constitutes acceptance of the updated terms. Such updates may be posted within the designated membership platform.
Client knowingly and voluntarily waives the right to a trial by jury in any dispute arising out of this Agreement.
This Agreement constitutes the entire agreement between the parties and supersedes all prior discussions, representations, or agreements, whether oral or written, regarding Union Puppy Club membership.
Client agrees that electronic signatures and digital acceptance of this Agreement shall be legally binding and enforceable to the same extent as a handwritten signature. Client further agrees that a copy of this Agreement bearing an electronic signature shall be deemed an original and admissible as evidence in any legal proceeding.
Client grants permission for photographs or videos taken during Union Puppy Club activities to be used for educational, promotional, or marketing purposes unless written opt-out is provided prior to use.
Your name and your dog’s name will only be used for identification purposes when appropriate. You may change or withdraw your consent at any time by emailing [email protected].
By signing below, Client acknowledges that they have read this Agreement in its entirety, understand its terms, and voluntarily agree to be legally bound by it. Client further acknowledges that they have had the opportunity to ask questions before signing and are signing this Agreement freely and without coercion. By signing this Agreement, Client confirms their enrollment in Union Puppy Club and agrees to comply with all terms, policies, and participation conditions associated with the membership.
